What are some of the causes of lack of communication in a relationship &…?

Obviously the causes are different for everyone but a list would be helpful. If we can identify the problem we'll be better able to reach a solution.
(I direct this question mostly to qualified psychologists / people who have studied some of this / or those who have overcome communication difficulties in their relationships !)
My problem is I jump to assumptions way tooo soon. Often times that assumption is wrong, if not completly off base. Because I have made that assumption, I find a way for everything to fit it. Its not a conscious process, but I catch myself doing it. And I have to step back and realy look at everything as a whole.
So I think not making assumptions if a huge one.
